As a member of NorthShore DPC, you gain access to comprehensive primary care services tailored to your unique health needs with a focus of preventive care and overall wellness.
Membership Pricing
Your membership is an annual subscription billed monthly. The annual membership subscription covers unlimited visits during business hours, extended appointment times, and the full range of services listed above, with no copays, hidden fees, or surprise bills. Please note that if you cancel your membership and later choose to rejoin, re-enrollment is subject to availability and includes a $350 re-enrollment fee to ensure continuity of care and access to our services. For non-members, we offer flexible options, including a one-time acute care visit and a one-time acute care package that includes a follow-up appointment. Non-member services are limited to acute care services only and not suitable for chronic disease management which requires continuous care.
